Coronavirus Self-Declaration

Due to the ongoing and rapidly changing situation with the novel-coronavirus (COVID-19) Hope Christian College is requiring all visitors to the College to complete this Self Declaration form.

Based on your answers provided below, Hope Christian College retains the right to deny you access to the site. We will work with you in an attempt to achieve your reason for being on site if access is denied.

Visitors to Hope Christian College must:

  • Sanitise or wash hands with soap and water for at least 20 seconds
  • Maintain a physical distance from other people on site of a minimum of 1.5m at all times where possible
  • Try not to touch your eyes, mouth and nose
  • Cover your nose and mouth with a tissue when you cough and sneeze. If you don’t have a tissue cough or sneeze into your upper sleeve or elbow

Each visitor much complete a form, each time they visit.

This data will be retained confidentially by the College for twelve months after submission.

The health and wellbeing of the College community is our first priority therefore Hope Christian College reserves the right to deny entry to the site.

    1. Do you have any of the following flu-like symptoms?





    2. Have you arrived in Adelaide from another Country or another State in the last 14 days?

    3. Have you had contact with someone from interstate/overseas in the past 2 weeks?

    4. Have you come into close contact with a confirmed case of the coronavirus, or someone from a "declared area" or who has displayed cold, flu or virus like symptoms in the last 14 days? (i.e. family, household members, friends)

    If your answer to the above is Yes, you should seek medical advice and will not be permitted on-site.
